The application must include a copy of the CV, transcripts, school’s medical malpractice insurance ($1,000,000 occurrence / $3,000,000 annual aggregate) and a copy of current personal health insurance. Incomplete applications will be marked as ineligible and will not be reviewed until the application is complete.

Foreign medical students must complete the same requirements as the US medical students. In addition, they must also include the CIE 701/702 form with their application. If your school holds the CIE 702 Form you are required to have Step 1 and Step 2 completed before the start of the rotation. This form is required by the State Commission for Independent Education for all foreign medical schools. Should your application be approved, final approval is contingent upon the State’s review and approval of this form. This form can be obtained by your school’s coordinator via the Florida Department of Education website.
