Orlando Health Home Care understands the importance of regular medication reminders and safety for your loved ones. Whether due to aging, disability, illness or injury, our registered nurses (RNs) or licensed practical nurses (LPNs) can provide a complete continuum of care at home.

Orlando Health House Calls
For several years, Orlando Health’s House Calls team has been extending primary care services to patients in the home for direct care and care management. House Calls can serve as your primary care provider or can bridge the gap between hospital discharge and physician appointments, allowing your care to be expertly managed until your primary physician can see you. Services provided include, but are not limited to:
- Management of chronic conditions, such as diabetes, high blood pressure, infections and injuries
- Prescribing medications and other treatments under the guidance of a physician
Medical Social Services
We provide medical social services to help guide families through the healthcare maze. We can aid in accessing outside resources, improving care management and supporting caregivers. Through coordination with many community-based organizations, assistance includes:
- Crisis intervention
- Consultations and counseling services
- Connecting you with networks and organizations, such as Meals On Wheels
Those patients eligible for home health skilled services may also receive assistance with personal care from a home health aide.
Home health aides assist with activities of daily living and related health services, such as:
- Bathing/hygiene needs
- Help with preparing specialized diets or meals
- Follow-up of therapy exercises for faster healing and greater comfort
Private Duty Services
Our private duty services can be a viable option for those who want to age at home versus assisted living or nursing home facilities; everything from personal care to medication management can be provided in the home 24 hours a day, seven days a week. A customized plan is developed with patient- and family-directed goals. Services are usually paid with out-of-pocket funds. Private duty services are ideal for maintaining independence, assistance with caregiving duties, or providing respite for the caregiver. Private duty services can provide:
- Bathing and hygiene assistance
- Help with dressing
- Meal preparation
- Light housekeeping and laundry
- Exercise and walking
- Interaction activities for mental stimulation
Rehabilitation Services at Home
Our rehabilitation services allow you to start therapy or continue rehab after hospital discharge in the convenience and comfort of your home. Using an interdisciplinary team approach, we work together to help you meet or exceed your individual rehabilitation goals through every stage of recovery. This medically necessary care is ordered by a physician, and our therapists work closely with your physician to achieve therapy goals. Our rehabilitation services include:
- Physical therapy
- Speech therapy
- Occupational therapy
- Lymphedema therapy
We offer respite services for primary caregivers whenever needed, whether for a few hours, overnight or 24-hour extended care. Our expert and compassionate care provides families with peace of mind, knowing that your loved one is being well cared for while you are away. Respite services allow caregivers the opportunity to focus on their needs, including:
- Spending time with other friends and family
- Running errands
- Attending church services or social events
- Enjoying a night out
- Traveling out of town

Home Care Helps Reduce Re-admissions

If you or a loved one needs medical monitoring but wants to remain at home, our telemonitoring program can provide the solution. For example, if you are chronically ill or recovering at home after a hospital stay, our telemonitoring program can connect with you to assess your health status and talk with you about how you are feeling.
Our telemonitoring technology offers tips and tools to better manage a chronic condition, such as heart failure or diabetes. You will receive education and guidance from a nurse specialist on how to make behavior modifications and lifestyle choices that will allow you to reach or maintain an optimal quality of health. Individualized programs are developed with your physician to provide monitoring of vital data, such as:

- Daily weight
- Blood oxygen levels
- Blood glucose levels
