Home Care Helps Reduce Re-admissions
Did you know that 20 percent of Medicare patients are readmitted to a hospital within one month of being discharged? Orlando Health Home Care understands the value of reducing hospital readmission rates, specifically for acute myocardial infarction, congestive heart failure and pneumonia.
Our skilled nurses understand how to monitor and report symptoms of worsening or recurring:
- Acute myocardial infarction
- Congestive heart failure
If you or a loved one needs medical monitoring but wants to remain at home, our telemonitoring program can provide the solution. For example, if you are chronically ill or recovering at home after a hospital stay, our telemonitoring program can connect with you to assess your health status and talk with you about how you are feeling.
Our telemonitoring technology offers tips and tools to better manage a chronic condition, such as heart failure or diabetes. You will receive education and guidance from a nurse specialist on how to make behavior modifications and lifestyle choices that will allow you to reach or maintain an optimal quality of health. Individualized programs are developed with your physician to provide monitoring of vital data, such as:
- Heart rate
- Blood pressure
- Daily weight
- Blood oxygen levels
- Blood glucose levels
