You need to bring a picture ID and current insurance card to your appointment.

We recommend you arrive at least 15 minutes before your appointment time to complete the check-in process.

For HMO patients, we ask that you verify with your insurance company that our physicians have been registered as the primary care physicians (PCP) before arrival. If we are not listed as such, you will be considered self-pay for that visit.

Medicare HMO and Advantage Plan patients, please bring all specialists’ names, addresses, future appointment dates and records available to your first appointment to ensure a smooth referral process.
