Our interdisciplinary teams are attentive and collaborative in their ability to individualize a plan of care for persons who have had joints replaced, as well as having ongoing non-related medical complications. These might include:

  • COPD
  • Diabetes
  • Cardiac concerns
  • Stroke survivors
  • Amputations
  • Osteoarthritis of other joints and limbsGout or other medical challenges

Our team will work with patients and families to create a program that allows maximum participation and encourages patient ownership of their rehabilitation process. Our social workers and care coordinators provide emotional support and guidance, our therapists excel with patient motivation and pain management strategies, our nurses closely monitor each patients’ medical stability and our clinical nursing assistants provide diligent attention to patient needs. Upon discharge from inpatient rehabilitation, we offer coordinated care with our outpatient rehabilitation team and home care.
