    You will only receive a bill from us if your insurance company denies payment or if you have a co-payment or deductible due. We accept payment by mail and over the phone for credit/debit card payments. To make a payment or for billing questions, please call 1-800-475-6112 In the event you are unable to speak with a representative when you call, please leave a message with the patient’s name, account number (if known), the date of service, the reason for your call, and the phone number where we can contact you. We will return your call within the next business day.
