Amenities & Resources at Orlando Health Horizon West Hospital
The Registration office at Orlando Health Horizon West Hospital is open 6:00 a.m. to 5:30 p.m. in the hospital lobby on the first floor if you wish to register in person. Registration in the Emergency Room is open 24/7. You will need the following items to complete your registration:
- Your doctor’s written or printed order with diagnostics (this does not apply to ER patients)
- Your driver’s license or state I.D.
- Your insurance card
Please be prepared to also provide:

Your contact information. We will need your social security number, address and telephone number.

Your insurance information. If you have insurance but do not have a copy of your insurance card, we will need the following information to bill your insurance: the name and mailing address of your insurance provider, your policy’s group number, and your personal identification number. If your insurance requires a referral, please bring the referral form to your first appointment. If you’re covered by Medicare or Medicaid, please bring the appropriate cards.

Your medical history. If you’re coming for a second opinion or on a physician referral, you may need to bring medical records, X-rays, lab results, pathology slides or other items. Your doctor may have already sent these to us – be sure to check with them before your appointment.

Advance directive. If you have an advance directive (such as a living will and/or durable power of attorney for healthcare), please bring it with you.
What you should leave at home:
- All jewelry and other valuables (we cannot be held responsible for personal items that are lost or stolen on our premises)
- Money and credit cards
- Electrical appliances such as hair dryers, electric shavers, radios, etc.
For extended stays, you may bring:
- Personal toiletries
- Bathrobe
- Slippers or socks
- Medications you routinely take (write your name on the containers and let your nurse know your medication routine)
- Eyeglasses
- Dentures
- Hearing aids
Patients are allowed two adult visitors (18 years or older). All visitors will continue to be screened and should wear masks and maintain social distancing. Limited visitation is still in place for hospitalized COVID-19 positive patients. No visitors are allowed after 9:00 pm except for emergent situations.
Hospital chaplains offer a safe place for you to discuss your concerns and fears and decrease stress. Our chaplains are clinically trained to provide spiritual and emotional support to our patients, guests and staff. They are available 24/7 to help with:
- Bereavement support
- Family meetings
- Communicating a patient’s spiritual needs with their medical team
- Contacting religious leaders from a patient’s faith tradition
- Education about living wills and health care surrogate documents
- Sacraments and religious rituals
- Seeking inner peace and strength
You may visit the hospital’s interfaith chapel – located on the first floor between the Gift Shop and the Cafeteria – at any time of day. To have a chaplain visit a hospital room, please call the hospital operator (0) and request to page the chaplain on duty. Or ask a member of your medical team to call a chaplain for you.
Located on the far right end of the hospital’s first floor, Orlando Health Horizon West Café provides healthy meals and snacks for patients, staff and guests. Menu options include freshly made brick oven pizza, grill items, a specialty pop-up station featuring fresh local ingredients, soup, salad and more. Cash and credit cards are accepted.
Service hours are:
Monday – Friday, 7:00 am to 5:00 pm
Saturday and Sunday, 7:00 am to 2:00 pm
Patients are welcome to order food from the café 7 days a week from 7:00 am - 7:00 pm.
If you would like more information about any of our services, we invite you to call Guest Services at (407) 407-1250. You may also stop by our Guest Services desk located in the lobby just inside the main hospital entrance. A Guest Services representative is available there from 8:00 am to 4:30 pm to help you in any way they can.
Request Medical Records
If you need copies of your or a loved one’s medical records, the Release of Information Department at Orlando Health can help process your request. The Medical Records office at Orlando Health Horizon West Hospital is also open Monday - Friday, 8:00 am - 4:30 pm for any walk-in requests.
