How To Apply

If you are interested in applying to our program, the following documents need to be submitted for review:

Primary Care Sports Medicine ERAS Fellowship Application

• Current CV
• Three (3) Letters of Recommendation (one from your program director)
• Official medical school transcript
• Official USMLE/COMLEX transcripts
• Copy of medical school diploma
• ECFMG certificate (if applicable)
• Personal statement
• Current medical license

The fellowship is ACGME-accredited through the Orlando Health Department of Pediatrics,although we are accredited through the department of pediatrics, we accept residents from family medicine, internal medicine and emergency medicine. 

The PCSM NRMP code is 1107333F0.
