Services and Specialties
The Orlando Health Support Team for Aftercare and Resources (STAR) Outpatient Centers are designed with two separate suites to provide coordinated care to patients.
The aftercare services suite, Suite A, is focused on offering health and wellness services for patients after they are discharged from the hospital. Patients can be referred to Suite A at the centers if they are unable to see their primary care doctor within seven days after discharge or if they do not have a primary care doctor
The medication disease management suite, Suite B, is focused on improving care for patients on specialty, complex, and/or high-cost medications. Patients can be referred to Suite B at the centers if they have qualifying medical conditions, are prescribed specific medications, and are on an eligible health plan.
The STAR Outpatient Centers have two convenient locations on the campuses of Orlando Health Dr. P. Phillips Hospital and Orlando Health Orlando Regional Medical Center.
