Support Team for Aftercare and Resources (STAR) Outpatient Centers

The Orlando Health Support Team for Aftercare and Resources (STAR) Outpatient Centers offer health and wellness services for patients after they are discharged from the hospital. Patients can be referred to a center if they are unable to see their primary care doctor within seven days after discharge or if they do not have a primary care doctor.

Services provided include:

• Post-discharge exam
• Medication management
• Social services
• Care management services
• Disease prevention education
