The STAR Centers are short-term care centers designed to support patients after a hospital discharge. Our goal is to help you transition safely back into the community, address your immediate healthcare needs and assist you in finding a long-term primary care provider (PCP).
You may be scheduled with a STAR Center if:
- You do not have a primary care provider (PCP)
- You cannot get a timely appointment with your current PCP after being discharged from the hospital
- You need support managing your medical care post-hospitalization
During your visit, our healthcare team will:
- Review your hospital course
- Assess your current symptoms or concerns and prescribe new medications as needed
- Go over your medications and adjust dosing if needed
- Update or clarify your plan of care
- Help connect you to a long-term PCP who accepts your insurance and fits your preferences
No, the STAR Center is not meant to be a long-term solution. We are a bridge to long-term care, and our priority is to get you established with a PCP who will manage your ongoing healthcare needs.
Our team will help guide you through this process. We will consider your:
- Location
- Insurance coverage
- Language preferences
- Special healthcare needs
No. We work with all patients regardless of insurance. Our team also can assist with connecting you to financial assistance resources if needed.
Please bring:
- Your discharge paperwork from the hospital
- A list of your current medications (or bring the bottles)
- Your insurance card (if you have one)
- Any questions or concerns you have about your health or care plan
Yes, we can help review your prescriptions and provide short-term refills until you are connected with your PCP.
If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. For non-emergency needs, you may contact the STAR Center for assistance with scheduling an earlier appointment to help manage your symptoms.
We strongly recommend being seen by a provider within seven days of hospital discharge to ensure your recovery is going as planned. If you already have a PCP but cannot get an appointment in time, we can still see you for short-term care.
