Enrollment
Getting started is simple. Patients can enroll directly or be referred by a healthcare provider to participate in the Maternal Telehealth Program. Once a referral or enrollment request is received, our care team will review eligibility and reach out to guide you through the next steps. Whether you’re a provider coordinating care or a patient seeking additional support during pregnancy or the postpartum period, our team is here to ensure a smooth, timely enrollment process so you can begin receiving care as soon as possible.
Appointment Schedule
You will have an appointment schedule with a program telecare nurse, which includes:
- Telehealth visits starting the first week after enrollment
- Virtual meetings at least once a month (more often at first)
- Follow-up visits at least once a month for a full year after your delivery experience
During the telehealth visits, the nurse will help monitor your progress and check for potential complications. This will involve asking some personal questions about your circumstances to better support you throughout your pregnancy, as well as your healing and recovery after delivery. You will be asked about:
- Physical symptoms (for example: Are you experiencing any limitations? Is there any bleeding?)
- Mental, emotional and psychological symptoms (for example: Are you experiencing mood swings? Do you have a history of anxiety, depression, eating disorders or other mental health conditions?)
- Social determinants of health – These are non-medical factors that affect your daily living conditions (for example: limited access to adequate food, housing instability or unreliable transportation).
Note: Identifying needs or concerns during an appointment is okay. To provide you with the best possible support, this may result in more frequent telehealth visits.
