Whether you’re preparing for a hospital stay or visiting a family member who is a patient, knowing the below helpful information will help make your visit less stressful.
If you wish to register in person, the Orlando Health St. Cloud Hospital Admitting/Outpatient Registration office is open Monday – Friday, 6:30 am – 6:30 pm and Saturday, 7:00 am – 3:00 pm. The ER Registration office is open 24 hours a day, seven days a week. You will need to have these items to complete your registration:
- Doctor’s written or printed order with diagnosis (unless ER patient)
- Driver’s license
- Insurance card
What you should bring:
- Contact Information — You will need to provide us with your social security number, address and telephone number.
- Insurance Information — If you have an insurance card, bring it with you. To bill your insurance, we will need the name and mailing address of your insurance provider, your policy’s group number and your personal identification number. If your insurance requires a referral, please bring the form with you to your first appointment. If you are covered by Medicare or Medicaid, please bring the appropriate cards.
- Medical History — If you are coming in for a second opinion or on a physician referral, you may need to bring medical records, X-rays, lab results, pathology slides or other items. Your physician already may have sent these to us — be sure to check with them before your appointment.
- Advance Directive — If you have an advance directive (such as a living will and/or durable power of attorney for healthcare) be sure to bring it with you.
- If you have an extended stay, you can bring the following things with you: o Personal toiletries, bathrobe, slippers or socks. o Any medications you routinely take. Be sure to write your name on the containers and let your nurse know (a list of all your current prescriptions is also helpful). o Eyeglasses, dentures, hearing aids, etc. o Any written instructions from your physician.
What you should leave at home:
- We ask that you leave all valuables at home or with a family member when you are admitted for inpatient or outpatient care. We cannot be held responsible for personal items that are lost or stolen on our premises.
- Jewelry and other valuables.
- Money and credit cards (although you may want a few dollars to buy a snack).
- Electrical appliances (e.g., hair dryer, electric shaver, radio).
Orlando Health St. Cloud Hospital is participating in a Medicare initiative called the Comprehensive Care for Joint Replacement (CJR) model. The CJR model aims to promote quality and financial accountability for care surrounding lower-extremity joint replacement (LEJR) procedures, commonly referred to as hip and knee replacements and/or other major leg procedures.

Selection Criteria for Collaborators
- General Provider and Supplier Collaborators (including Physicians). Orlando Health St. Cloud Hospital will select CJR collaborators in a manner consistent with the CJR Rule and based on quality performance criteria and participation in care redesign (i.e., collaborators will be required to participate actively in care redesign for improving and enhancing the care and management of care for beneficiaries in a CJR episode). The selection criteria shall not be based directly or indirectly on the volume or value of referrals or business otherwise generated by, between or among Orlando Health St. Cloud Hospital, CJR collaborators, and any individual or entity affiliated with Orlando Health St. Cloud Hospital or CJR collaborator. Any collaborator selected for participation in the CJR Program must execute a collaborator agreement with Orlando Health St. Cloud Hospital setting forth the applicable elements of the CJR Program and the sharing arrangement in order to be eligible for gainsharing payments.
- Physician Collaborators Additional Requirements. For physician collaborators, the selection criteria for participation in the CJR Program shall include the following requirements:
- Collaborator must be an active member of the medical staff in good standing;
- Collaborator must hold clinical privileges to perform episode based procedures in the CJR Program (DRG 469 and 470) as may be amended from time to time by CMS
- Collaborator must be a participating provider in the Medicare program;
- Collaborator must be in compliance with all Medicare provider enrollment requirements (set forth at 42 C.F.R. Sections 424.500 et seq.);
- Collaborator must have executed a physician collaborator agreement with Orlando Health St. Cloud Hospital and not be in breach of such agreement or any representation and warranty made in such agreement;
- Collaborator must agree to participate in care redesign activities related to the CJR Program;
- Collaborator must have and maintain a valid and active tax identification number and NPI;
- Additional selection criteria may be approved by the Committee. The selection criteria for physician collaborators shall not be based directly or indirectly on the volume or value of referrals or business otherwise generated between Orlando Health St. Cloud Hospital and such collaborator physicians;
- Collaborator must not be excluded from participating in Medicare, Medicaid, or any other federal healthcare program, as defined at 42 U.S.C. Section 1320a-7b(f).
Orlando Health St. Cloud Hospital is located in St. Cloud, Florida, about 30 minutes from downtown Orlando and 25 minutes from Orlando International Airport. In close proximity to Kissimmee and Harmony, the hospital also is near Lake Nona, Buenaventura Lakes, Narcoossee and Hunters Creek.

Physician Billing Information
During your care at Orlando Health St. Cloud Hospital, you may receive services from various medical providers. You will receive a separate billing from some of these medical providers, such as ER physicians, anesthesiologists, radiologists and pathologists. Please contact them directly if you have any questions concerning those bills you receive.
To request copies of medical records/health information for care received at Orlando Health St. Cloud Hospital, please call Orlando Health St. Cloud at (407) 498-3314 and ask for the “Authorization for Release, Use and Disclosure of Health Records” medical record request form. You can also request medical records within the Patient Portal. You will need to bring the completed form and a photo ID to the hospital in order to obtain your records.
