It’s important that you continue your care after you leave the hospital by taking any prescribed medications, eating as your doctor has advised, arranging for services or equipment needed, and following up with your physician as recommended. Your personalized information for these follow-up activities can be found in your After Visit Summary paperwork, along with a work/school release letter. Please review this information and be sure to schedule follow-up appointments or at-home care.

Use MyChart to Stay Connected After You Leave the Hospital

You may use MyChart to access your After Visit Summary, manage appointments, view test results, message your provider, request prescription refills, pay bills and more – all from your phone, tablet or computer.
