Do GLP-1 Weight Loss Drugs Affect Birth Control Efficacy?

It’s easy to get excited about GLP-1s as the answer to weight loss, but before committing to this medication, you should understand how their side effects can make birth control pills less effective.
What Are GLP-1s and How Do They Work?
Glucagon-like peptide-1, aka GLP-1, is a hormone that occurs naturally in your body. It’s released after you eat to signal fullness. It is prescribed for weight loss as it slows gastric emptying, so people feel fuller longer. These medications also send signals to the GLP-1 receptors, acting on the parts of the brain that sense satiety, limiting your desire to eat more.
There are currently six versions of GLP-1s, with three typically prescribed for weight loss.
- Semaglutide
- Tirzepatide
- Liraglutide
GLP-1s aren’t just prescribed for weight loss. They’re also prescribed to help manage several diagnoses.
- Type 2 diabetes
- Metabolic dysfunction-associated steatotic liver disease (MASLD)
- Obstructive sleep apnea
Why GLP-1 Side Effects Can Lower Birth Control Efficacy
Oral contraceptives contain progestin, a synthetic hormone that has several roles:
- Suppresses the hormones that signal to the ovaries to release an egg, helping prevent pregnancy
- Thickens cervical mucus to make it more difficult for sperm to fertilize an egg
- Thins the endometrial lining to help make pregnancy less possible and reduces or ceases any menstrual bleeding
The biggest possible side effects of GLP-1s are vomiting and diarrhea. If you throw up shortly after taking your pill or have diarrhea that makes the birth control pill pass through your system too quickly, you aren’t getting the full dose. That increases your risk of an unintended pregnancy.
Tirzepatide might be the biggest risk because it slows down gastric emptying, leading to a smaller absorption window for your birth control than other GLP-1s. You or your partner should use a backup form of contraception during the period that you remain on tirzepatide.
Can GLP-1s Increase Fertility?
For women who want to be mothers, the good news is that GLP-1s can help regulate hormones, which can lead to improved fertility. Being significantly overweight can negatively impact hormone production, which can make getting pregnant difficult. Losing enough weight to return to suggested healthy ranges can help restore normal hormone production, leading to regular menstrual cycles and increased fertility.
Non-Oral Birth Control Alternatives for GLP-1 Users
If you are on GLP-1s and oral birth control, you may want to discuss with your primary care doctor what the best ways are to help manage the issue. Oral contraceptives are a convenient birth control method, but they aren’t the only option. Any of the following non-oral methods won’t be impacted by these possible GLP-1 side effects.
- Hormonal IUDs
- Copper IUDs
- Birth control implant
- Birth control shot
- Birth control patch
- Vaginal ring
- Barrier methods such as condoms or diaphragms
You can consult your doctor along your GLP-1 journey. Every person responds to medication slightly differently. Some women might experience significant side effects from GLP-1s, and others won’t have any. If you are experiencing side effects, you have several options, including changing birth control methods, using a back-up method in tandem or choosing a different type of GLP-1 that has fewer side effects. Meeting with your primary doctor can help you decide the best path forward.
