About Orlando Health House Calls

Our Orlando Health House calls program is specifically designed for patients who can’t make it to one of our clinic locations for heart failure care. House calls are typically longer than a provider office visit, allowing the provider to completely customize the information shared and assessments made during the visit. Using this service can help reduce your risk of needing hospitalization for heart failure care since we continually keep up with your care. We currently offer House calls in Orange, Seminole and Osceola counties.

During a House calls visit, a physician or nurse practitioner can:

  • Diagnose, treat and manage heart failure and related symptoms
  • Prescribe or refill heart failure medications and other treatments
  • Order, perform and interpret diagnostic tests, including lab work, X-rays and ultrasounds
  • Evaluate if you need durable medical equipment, home care or wound care, as needed

How House Calls Work

You’ll be referred to our team for a House calls visit in one of two ways:

  • At hospital discharge. If you have been hospitalized for heart failure at an Orlando Health hospital and don’t currently have a primary care provider, you may be referred to Orlando Health House calls. The Orlando Health team can continue to closely monitor and manage your heart health through these visits, reducing your risk of hospital readmission due to heart failure.
  • By the Advanced Heart Failure Clinic. If you have been seen by the Orlando Health Advanced Heart Failure Clinic and they believe you may benefit from more frequent, in-home care, you may be referred to the House calls team.
